A PATIENT REGISTRATION FORM and a MEDICAL INFORMATION RELEASE FORM are available on this site for use by both new and existing patients. Click on the Printable Documents link that appears in FAQ link in the navigation bar on the left side of this home page to learn more about these documents. To access a document, go to the Patient Forms section in the FAQ page and click on the document of your choice. LOG IN TO OUR SITE and we will register you and give you your own "my patient page" so that you may send us general messages (including requests for appointments and prescription refills), as well as send secure, confidential emails directly to Dr. Scalettar. You also will be able to create your interactive personal health record and access your email (including appointment confirmations and reminders) sent to you from our office. Once we have registered you, you will receive an email telling you that you then are able to use all the site's features, including the email messaging feature.
